Pediatric Ankle Trauma Treatment : CFAS Patient Testimonial

Video Transcript & Case Summary:

Foot Specialist: Recently, we treated one of our youngest patients who suffered a very serious tibia fracture. The fracture was located extremely close to her growth plates, which immediately limited our traditional surgical treatment options. If we were to use standard plates and internal screws, we risked causing irreversible damage to the growth plate, potentially leading to future limb length discrepancies or angular deformities.

To avoid this, we successfully treated her broken leg using an Ilizarov external fixation device. This specialized technique allowed us to completely stabilize the shattered bone from the outside, utilizing tensioned wires that safely bypass the delicate growth plates. The frame held the tibia in perfect anatomical alignment while the bone healed naturally. Today, we are thrilled to say the fixation was a success, her growth plates remain healthy, and she is fully back on her feet!

Why Are Fractures Near the Growth Plate So Dangerous?

In children, the physis (growth plate) is the area of developing tissue near the ends of long bones. It is the weakest part of the growing skeleton—often weaker than the surrounding ligaments and tendons.

When a severe trauma or fracture occurs near the ankle or lower tibia, standard treatments (like inserting large metal plates and screws directly into the bone) can permanently fuse or damage the growth plate. This can result in:

  • Premature Growth Arrest: The affected leg stops growing, resulting in one leg being shorter than the other.
  • Angular Deformities: The bone grows crooked, leading to gait abnormalities and chronic pain.

To prevent these complications, our clinical team utilizes advanced external fixation.

What is Ilizarov External Fixation?

The Ilizarov apparatus is a highly specialized, circular external fixator used in orthopedic and podiatric surgery to treat complex fractures, lengthen bones, and reshape limb deformities. Named after the surgeon who invented it, the device consists of external rings connected by threaded rods and tensioned wires that pass directly through the bone.

Benefits of the Ilizarov Method for Pediatric Patients:

  • Protects the Growth Plates: Because the device is external, the tensioned wires can be strategically placed to completely bypass the child’s growth plates, ensuring natural bone growth continues after the fracture heals.
  • Superior Stability: The circular ring design provides 360-degree, three-dimensional stability to the fractured tibia, allowing for highly precise alignment of the bone fragments.
  • Encourages Faster Healing: The apparatus allows for micromotion at the fracture site, which actually stimulates bone healing and regeneration.
  • Early Mobility: Unlike traditional heavy casts where the patient is completely bedridden, the Ilizarov frame often allows patients to bear weight and walk much sooner during the recovery process.
